The partners in this community service activity were participants of the Chronic Disease Management Program (Program Pengelolaan Penyakit Kronis/Prolanis) from three village community centers within the working area of Pakisaji Public Health Center. Prolanis participants are a community group requiring strengthened knowledge and awareness regarding efforts to prevent chronic diseases and their complications, including osteoporosis. The identified problem was the participants’ limited knowledge of osteoporosis, particularly regarding its definition, risk factors, and preventive measures. This condition may affect the participants’ ability to adopt behaviors that support bone health and reduce the risk of osteoporosis. This community service activity aimed to improve Prolanis participants’ knowledge of osteoporosis and its prevention through health education. The activity involved 70 Prolanis participants from three village community centers within the working area of Pakisaji Public Health Center and was conducted in July 2026. The health education was delivered through interactive lectures, discussions, and question-and-answer sessions using PowerPoint and leaflet media. Participants’ knowledge was assessed using pre-test and post-test questionnaires covering the definition of osteoporosis, risk factors, and preventive measures. The data were analyzed descriptively, followed by the Kolmogorov–Smirnov normality test and the Wilcoxon Signed-Rank Test. The contribution of this activity was to provide health education as a means of strengthening participants’ understanding of osteoporosis risk factors and preventive measures. The results showed that the mean knowledge score increased from 64.57 to 92.29, representing an increase of 27.72 points. The Wilcoxon test showed a statistically significant difference between the pre-test and post-test scores (p < 0.001). A total of 58 participants (82.9%) experienced an increase in their scores, 12 participants (17.1%) had unchanged scores, and none experienced a decrease in their scores. Therefore, health education was effective in improving Prolanis participants’ knowledge regarding osteoporosis prevention.
